What to Bring to Your Appointment
- Your completed health history form
- Pertinent information about your medical history
- A list of current medications, vitamins or supplements you are
taking, including dose and frequency
- A list of any questions you want to ask

Patient Forms and Information Sheets
Please download the health history questionnaire and complete it before
arrive for your appointment. If you're unable to print the health
history form, please arrive 15 minutes early to your appointment in
order to complete the paperwork.
To print the below forms you must have Adobe Acrobat Reader on
your computer. You can download this program free of charge by clicking
on the above link. If you already have the Adobe Acrobat Reader, click
on the item you want to view and print.
Instructions

What to Expect
- An initial acupuncture treatment generally lasts 1.5 hours.
- Follow-up treatments are one hour.
- A diagnosis includes taking the pulse, palpating the neck and abdomen, looking at your tongue as well as reviewing your health history form.
- Treatment is based on the comprehensive assessment from all the collected information.
- Only sterile, disposable hair thin needles are used.
- Sensations felt upon insertion include a slight pinch, tingling or a zing of electrical energy or Qi.
Once the needles are in you might feel heat, heaviness or an achy feeling at the site of insertion.
This feeling usually dissipates shortly after the needles are removed.
- Treatments are generally very relaxing, with many people falling asleep during the treatment.
Depending on the individual, results may be experienced immediately or may take several treatments to achieve the desired effect.
- Usually treatments occur 1-2 times per week, depending on the condition.
- Please wear comfortable loose clothing, shorts or pants that can be raised above the knee. Women should wear a sports bra and avoid dresses. If you need to undress to reach particular areas a sheet or blanket will be provided.

Fee and Payment Information
An initial session is $75 and follow-up sessions are $60.
We request payment at the time of your appointment.Generally insurance
does not cover acupunture treatment except for personal injury i.e. car
accidents.
